V. A statement on the authenticity of the notice: "I guarantee that the information in this notice is full, true and accurate. If the content of this right notice is not completely true, I/the unit will bear all legal responsibilities arising therefrom.

VI. Sign the document and stamp with the official seal for agency or organization established according to laws.
